Incorporating ActiveX Upload Control for Web

I want to incorporate the ActiveX Upload Control into my form that will be served up to the web. Where do I get the control and how/where do I put it into my form?

While in design mode, place the cursor wherever you would like to insert the File Upload control and then use the following menu choices to insert the control....
MENU: Create Embedded Element File Upload Control This will insert a File Upload Control wherever your cursor is positioned. You can use the extended HTML attributes via the Properties InfoBox to give a NAME Tag to this item so you can reference it with JavaScript and perform any validations or actions necessary.
