Mailboxes losing icon

Sometimes, some of the mailboxes "lose" their icon (i.e., it is not shown and 'defined values' and 'updated by' are blank) and I must change it manually -- a design refresh or replace doesn't add/update the icon. When I make the change, the icon becomes visible and the above values are set. It has as default "Do not allow design refresh/replace to modify this design element."

Why does this happen and is there a more elegant and faster way to solve the problem? My company uses R5.011 and the template is from an older 5 version, but functions on most mailboxes. The problem appears on single users' mailboxes on many servers. We have a HUB-configured Notes network.
Good one! I have never seen (or heard of) this. But there is some discussion on Notes.net about it. Go to www.notes.net / Forums / Notes Domino 4-5. Then search for "icon and disappear." You may find something helpful.
