Notes forms only work in Lotus Notes. To accomplish what you want, use a Domino version of the form and send the SMTP client a link to the Domino form.

You could send a MIME formatted email, which allows you to include the data in tables, with images, etc.
The only problem is that Exchange 5.5 doesn't use the same MIME standards as Lotus Notes.
To get around this in the past, I've sent emails with the data formatted or anything else inside an HTML attachment. Unfortunately, using this solution, the form can not be submitted back to Notes -- if that was the intention.
