E-mail addresses in editable text fields

I am trying to create a form where the user enters his/her e-mail address into an editable text field. I then want to do a lookup to the names.nsf to see if that e-mail address exists. If it does, I want the other fields on the form to be automatically populated (i.e., FirstName, LastName, etc.). If it does not, then I want the fields FirstName and LastName to be editable text fields where the user enters the info. This seems to be an easy thing at first thought, but I have not found a solution.
This does work but is a bit of a task to get working the first time. First, you'll need to have (at least) read access for the user to the NAB. You'll also need to set the database properties to "use javascript when generating pages." Do your lookup to the NAB and use "refresh field on keyword change" in your form fields. You'll also need to drop the returned name information into hidden "select" fields and then use JavaScript to populate "div" areas to display the name. Hint: the HTML "select" is saved to Notes text fields that will hold the name information.
