You can have forms for the Web and forms for Notes. Just create your two forms and then from the designer view, right click one and select "design properties". Then you can see that you can hide them from the web or notes from the design tab.
For your table problem, you can consider using a field to control your programmable table on the Web. What you have to do is Name the table and also Name the rows using the Name field in the Web tab of the table properties. If you name the table "MyTable" and named the rows "FirstRow" and "SecondRow" respectively, then all you would have to do to control your programmable table is to create a field on the form called "$MyTable" and have it evaluate to the name of the row you would like to display. The $MyTable field could be a radio button, a keywords field that refreshes on change, a field that is set by another field or action.
