Missing default form for name change requests

I am an administrator of a Domino 6.0.3 cluster. When I get name change requests I get a "Cannot locate default form" error and I can't even forward this to another admin. What default form am I missing? I have tried to refresh and replace my design. I have tried to install from the admin disk all options available and still no luck.
When you say "get name change requests" I assume you mean that you are getting these in e-mails, rather than seeing them in the Admin Request (AdminP) database. In general, this error occurs when a Notes document (e-mail in this case) has no form field or the form specified in the form field does not exist. When this happens, Notes looks in the database for a form that is marked as the "default form." It appears that you do not have a default form. So use Domino Designer to look at the list of forms in your mailfile. One of them (usually Memo) should have a blue arrow next to it, to indicate that it is the default form. If not, edit the Memo form, go to Form Properties and check the option to make it the default form.
