Phone number in address book

I would like to be able to take a phone number out of the name and address book from the name that they type in the name field in the same form. Technically this is a by the book formula. However when I do an @DBLookup, it states that it cannot find the index for that view. I am taking it from the people view in the Domino Directory. I am using R5, however I did try this on a 4.5 server as well and got the same results. Please help me solve this very frustrating problem.
Actually, the people view is probably not your best choice. The names in 
the people view  appear in the format: 'lastname, firstname'.  My name 
would appear only as 'Mahoney, Brian' .  I would use the view 
"($NamesFieldLookup)". this view lists people by variations of their name. 
I appear under three entries: "brian mahoney";  "brian mahoney/domain"; and 
"bmahoney".  This view is more likely to find the name as it is entered in 
the field. 
Additionally, I would wait to do the lookup until the document is being saved, so that you know the name field has been populated. A possible formula for the form's phonefield might be: @IF( @IsDocBeingSaved; @DbLookup ("" : "NoCache" ; "" : "names.nsf" ; "($NamesFieldLookup)" ; formsnamefield ; NABphonefield ); formphonefield )
